In an effort to reduce identity theft, the Federal Trade Commission has implemented a policy requiring businesses to verify the identification of consumers.  In compliance with this policy, Sand Creek Animal Hospital will need to see the driver's license of all new clients at the time of their first appointment. Please print and complete the following Client Registration form and bring it with you to your first appointment.  Also, please fill out the Authorization for Release of Records and bring it with you so that we may obtain medical history from your previous veterinarian.
